Approval for State Certification Exams

Are you ready to take teacher certification exam? Log into Your UTA MyMav account and request approval using our online form at https://forms.office.com/r/sPaZVz00mu.

 

INSTRUCTIONS: Please make sure the information you enter is complete and correct. Allow 5 business days for approval. Approvals are issued during regular office hours. Your name must appear exactly as it appears on your valid state ID.

This form is for your first attempt at an exam. If you are requesting an exam retake, please email coedcertification@uta.edu for retake policy, which may include additional preparation, a study plan and Program Coordinator approval.
