To request permission to become an authorized driver of UTA vehicles, please complete and submit the Request for Review of Motor Vehicle Record (MVR) form to the Environmental Health & Safety Office (EH&S). A 3-year driving history must be secured with an acceptable rating. If a full 3-year history is not available from the Texas Department of Public Safety, then it is the employee’s responsibility to obtain a copy of their driving record(s) from any previous state(s) of residence.1045791
*Once an acceptable rating has been verified, all driver MVRs will be automatically checked regularly to ensure they maintain a satisfactory rating.
Please allow up to five (5) business days to process the request. Contact our office at ehsafety@uta.edu with any questions.
Employees must also complete the University's online Defensive Driving Awareness Training (DDAT) or a Defensive Driving Course (DDC) offered by an outside agency. The Defensive Driving Awareness Training is valid for 3 years and must then be repeated to remain an authorized driver. Individuals who have taken another accredited DDC within the past three years can be exempt from taking the UTA course by providing a copy of their DDC completion certificate. The UTA DDAT is not endorsed for ticket dismissal or an insurance discount.
The Defensive Driving Awareness Training can be accessed on the EH&S Training Management website. Log In/registration instructions are provided. However, if you encounter any problems, please contact EH&S at ehsafety@uta.edu, or 817-272-2185.
Operation of 12 and 15-Passenger Vans
12 and 15-passenger vans will only be operated by experienced, authorized drivers who understand and are familiar with the handling characteristics of the vans, especially when the van is fully loaded. In addition to the above requirements for approval to drive UTA vehicles, employees operating 12 and 15-passenger capacity vans must also complete van-specific training. Under no circumstances will an employee be approved or authorized to operate 12 and 15-passenger vans without fulfilling the acceptable 3-year driving history requirement.
The van training consists of an online course plus a behind-the-wheel road driving test. To access the 12 & 15 Passenger Van online course go to the EH&S Training Management website.
The van behind-the-wheel driving test will be conducted at the EH&S Office, 500 Summit Ave., on the dates and times listed below. Class size is limited. Drivers must have already completed the Defensive Driving Awareness Course, the online 12 & 15 Passenger Van Course, and have a current acceptable MVR check before they are eligible to participate in behind-the-wheel van training. If you have any questions, contact EH&S at 817-272-2185 or ehsafety@uta.edu.
